Build questionnaires and recurring check-ins, assign them to clients, and review what comes back.
Forms & check-ins is how you collect structured information from clients — a one-off intake questionnaire, or a check-in that repeats on a schedule. Open it from Forms in the left navigation.
Build a form
Create a form
Start a questionnaire (one-off) or a check-in (recurring), give it a title, and add questions. Question types include text, number, scale, yes/no, multiple choice, date, star rating, signature, and media (photo) capture.
Save it to your library
Mark a form as a library form to reuse it across clients instead of rebuilding it each time.
Assign and review
Assign to a client
Pick a client and a cadence — daily, weekly, every two weeks, monthly, or one-off. A questionnaire is one-off and unassigns itself once submitted; a check-in keeps recurring.
Review submissions
Submitted check-ins land in your review queue on the Forms hub. Open one to read the answers; mark it reviewed when you're done.
What clients see
A client opens an assigned form from Due now on their own Forms page and fills it in. Everything they've submitted is listed under Completed forms, tagged by type — Intake · One-off or Check-in · Weekly/Monthly — with search and filtering.
Tip
From the Completed forms list, Open renders any submitted form as a branded PDF that opens in the browser — ready to print or save.
Note
Visibility is enforced by row-level security: a client sees only their own submissions, and a coach sees a client's submissions only with a live coaching relationship.
