Create Intake Questionnaires

An intake questionnaire is a one-off form a client fills once at the start.

What you'll learn

1. Build the form

   In Forms, create a questionnaire and add questions — text, number, scale, multiple choice, date, signature, or photo capture.

2. Assign it once

   Assign it to a client as a one-off; it unassigns itself automatically once submitted.

3. Read the answers

   The submission lands in your review queue, and the client can re-open it as a PDF from Completed forms.
