Why your labs look fine and you still feel flat

A reference range is a statistical statement about a population, not a statement about you. It is built by measuring a few thousand people who were assumed to be healthy, then drawing a line around the middle 95 percent of them. Two and a half percent of well people fall below it. Two and a half percent fall above. And the whole width of the band is wide enough to hide a great deal of movement.

Normal means you are not unusual. It does not mean you are well.

This is why a result can sit comfortably inside the range and still be the most interesting number on the page. What matters is not where you sit in the population. It is where you sit relative to where you were.

Movement inside the band

Take ferritin. A man drifting from 180 to 95 over eighteen months is still inside every range he will be shown, and every report he receives will say normal. But he has lost nearly half his iron stores, and he will feel it long before a flag appears.

A value can fall by 40 percent and never leave the range.
Two labs can disagree on the range for the same marker.
The range often differs by sex and age, and sometimes by nothing at all.

What to do with a normal result that does not match how you feel

Ask for the number, not the verdict. Ask what it was last time. Ask whether the trend, rather than the value, would concern the person reading it. Those three questions turn a filed result into a conversation.
